Mountain Valley Health Care is a Medicare/Medicaid Certified agency dedicated to providing the residents of Northern Colorado with an exceptional choice of care at home. Whether it's our physical therapists helping an individual fully recover from a surgery, or providing bereavement support when a loved one dies, our fundamental belief is that healthcare is not just about medicine but also about relationships and trust.

Why have Home Care and Hospice care together?

Here in Colorado, as well as throughout the United States, Medicare certified skilled home health and hospice agencies are typically owned and managed independently of one another. These two types of health care providers have uniquely different goals and approaches for the patients and families they serve. Mountain Valley Health Care chooses to offer the two approaches within one agency. It can create challenges, but the benefits are far more rewarding. Who Pays for our Services?

Both home care and hospice services are generally covered by:
  • Medicare
  • Medicaid
  • Private Insurance
  • Long-Term Care Insurance
  • Private Pay

Medical Services must be authorized and ordered by your physician. Medical Services can be requested by the patient, the physician, hospital case managers, family members, friends and/or neighbors. Most Private Services require no physician authorization and can be requested by any and all of the above Call to have a no obligation consultation with a Nurse or Social Worker, so that you may explore all avenues available to you and your family.
