  • Medical Director - Works with the team to oversee the entire patient care process.
  • Nurses - Work closely with the patient's doctor, focusing on comfort and safety.
  • Medical Social Workers - Help coordinate care giving & planning, and provide anticipatory grief support to the family.
  • Certified Nurse Assistants - Assist in patient's personal care.
  • Chaplains - Offer spiritual support to patients and families.
  • Grief Support Staff - Facilitate a variety of bereavement services including Individual Counseling, Grief Support Groups, and Education.
  • Therapists – Physical, Occupational and Speech programs available to coordinate treatments with your nurse and Doctor

Developing an awareness of the issues involved in living the last months of life, coping with a life long illness or just trying to life live to the fullest, we view community education as an important part of our mission. We are eager to raise the awareness of everyone in the community regarding home health or hospice care. Bereavement Counselor Presentations

Grief 101  - Definitions of common experiences of grief. Factors that influence grief, tasks of grief and myths of grief.

Walking with Others Thru Grief – General helping strategies for walking with others through the grieving process, as well as Do’s and Don’ts.

Good Grief: Embracing the Journey  - Common experiences, tasks and factors that influence grief.

Children & Teenage Grief  - Age related needs and responses regarding children’s grief and loss. Symptoms of complicated grief in children.

When Parents Die: Responding to parental or secondary losses, reflections/journaling, and  building a framework for healthy response.

Grief in the Workplace  - Supporting co-workers and the effects of serious illness, loss, and grief in the workplace.

Holiday Grief  - Gives helpful hints on how to manage dealing with the loss of a loved one or friend during the holidays and special days like anniversaries and birthdays.
